Documents    

             1. A form of intention to support the Science and Technology Developement Fund.

             2. A cover letter from the company.

             3. A copy of company affidavit issued by the Ministry of Commerce (within 30 days).

             4. A copy of authorized person's valid ID or, in case of foriegn resident, copies of passport and work permit.

             5. A copy of Investment Promotion Certificate.
